Four error types account for hand-off failures: signal corruption (36.8%), data gap (29.1%), referential drift (27.3%), capability gap (6.8%).
The result reverses the common assumption. Close to two thirds of hand-off errors are transmission problems, not competence problems. The capability gap, the one most leaders assume first, weighs under 7%.

What it means for an SME running AI
Before funding more training, find out whether your problem is that people do not know, or that what they need never reached them. The two have almost nothing in common, and one is much cheaper to fix.
